Use of Technology as A Strategy to Improve Adherence to Treatment in Arterial Hypertension in Older People

ABSTRACT:
The accelerated demographic change that Chile is experiencing (18.4%, 2022) and longer life expectancy implies an increase in chronic pathologies such as Arterial Hypertension (30%), which is directly related with age. Adequate treatment avoids complications and maintains well-being and quality of life. However, poor adherence to treatment persists as a problem of concern for public policies. The Project seeks to improve adherence to treatment for hypertension in older people attending Family Health Centers of the commune of Hualpén, through the integration of the transmedial psychoeducational program to the Health Information System of the “Servicio de Salud Talcahuano” (Talcahuano Health Service). It is a correlational, quasi-experimental study with a sample of 320 older people, composed of two groups: patients intervened in 2018 and patients incorporated in 2023. The Morisky-Green instrument was used to measure pharmacological adherence and Biopsychosocial Instrument constructed by the research team. The results in a subsample of the group intervened in 2018 report a decrease in adherence by 2023 (54.8% to 45.2%), greater in women (51.3%) than men (33%), in people aged 60 to 79 years old (48.3%); with greater social support, health empowerment, family satisfaction, depressive symptoms (inverse) and use of Apps (Mann-Withney p<0.05). We conclude that the incorporation of technology by the elderly increases adherence to hypertension treatment; however, it is not sufficient on its own and requires systematic intervention by health care teams, incorporating an interoperable system integrated into the medical record system that also allows the enhancement of protective psychosocial factors for healthy longevity.
